Care Provisions AU are currently going through accreditation for registered service provision under the NDIS Quality and Safeguards Commission. At Care Provisions, we deliver complex case management under the new NDIS Mental Health recovery coaching structure. We have over 15 years of experience in community mental health nursing , existing and established community networks all pivoted by a person centered approach.
Our doors are open to new referrals. Our staff are dedicated professionals, with the expertise to deliver holistic, integrated services that make a powerful and sustainable difference to people’s lives.
If you require an experienced Support Coordinator or a Psychosocial Recovery Coach with in experience in complex case management, please go to our website and get in contact with us. We accept all referrals from participants, carers, family, organisations, advocates, DHHS, NDIS Planners and LAC's. We service all of Melbourne's Northern and Western regions.
